First-Time Patient Appointments
If you are a first-time patient, we request that you complete some necessary paperwork. If you would like to complete the paperwork before your appointment, feel free to download it here and fill it out prior to your arrival. If you plan to fill out your paperwork during the appointment, please arrive at least 15 minutes early so as not to alter our appointment schedule.
What to Expect at Your Appointment
Once your new patient paperwork is completed, one of dental hygienists will greet you and guide you into an X-ray room. Taking dental X-rays will help us evaluate any problems with your teeth that are invisible to the naked eye.
Once your X-rays are completed, your dental hygienist will ask you about the reason for your visit, any dental concerns you might have, and any goals you may have as it pertains to your oral health. He or she will also review your health and dental history.
Finally, your hygienist will perform a thorough dental cleaning and polishing. Our experienced dentists, Dr. Vaibhav Bajaj, will then inspect your teeth and jaw for any signs of damage.
At the end of your appointment, your dentist will share your treatment plan with the front office so that our staff can review any insurance and billing questions with you. We will then schedule your next appointment so that you are all prepared for your next visit.
Please arrive at least 15 minutes before your scheduled appointment time. This allows our staff adequate time to review your information and make other preparations. If you need to complete new patient paperwork, plan to arrive at least 20 minutes before your appointment.
Remember to bring your current insurance card; a list of medications, your driver’s license; and any dental records you would like to transfer, such as X-rays. Don’t forget your payment method– our office accepts cash, Visa, MasterCard, American Express, Discover, and CareCredit.
If you are not able attend your appointment, we require a 24-hour notice of cancellation. This enables us to accommodate our other patients. Patients who do not give proper cancellation notice may be subject to a cancellation fee.

We accept most forms of dental insurance, including Aetna DMO, and are happy to help you maximize your benefits.
